Tracking Community Outreach with Visits and Follow-Up Emails

Introduction
The client wanted a simple way to track their staff’s community outreach work. They were already using Salesforce to track injuries and treatments and wanted something similar for outreach.
Mission
The idea was to have a Community Outreach Partner record with basic contact information like address, phone number, email, and website. They also wanted to log visits to those partners, add notes, and be able to report on both partners and visits.
For example, if a staff member visited a local community center to provide educational materials, they could log the visit under that partner’s record, add notes about the conversation and any follow-up actions, and later include the visit details in outreach reports.
Solution
We have created two new objects: Community Outreach Partner and Visit. The partner record pulls in contact info from the related Party record. Staff can create a Visit linked to a partner and add notes as needed.
Later, we added a “Send Thank You Email” feature. When certain conditions are met, for example, when both the visit value and interaction score are 4 or 5 on a 1–5 scale, a button will appear on the partner record to send a thank-you email. Users can send a default template message or customize the email for their purpose. The email feature supports formatting and images, allowing users to create professional and personalized messages.
Results
The client can now track outreach work more effectively, log visits in a structured way, and send follow-up emails when the visit meets certain criteria. The process is simple, works within their existing system, and gives them better visibility of how they connect with the community.
